As the prevalence of obesity and metabolic dysfunction-associated steatotic liver disease (MASLD) continues to rise, accurate non-invasive assessment of liver disease remains a critical clinical challenge.

A recent study involving 245 patients with MASLD who underwent both FibroScan assessment and liver biopsy found that FibroScan significantly overestimated advanced fibrosis and cirrhosis in patients with class 3 obesity. The investigators concluded that, because of the impact of severe obesity on test performance, a second non-invasive test may be warranted before making treatment decisions or excluding patients from liver-directed therapies.

These findings underscore an important reality in liver disease management: patient characteristics can influence the performance of some diagnostic approaches, making accurate staging more challenging in certain populations.
